
A new systematic review and bayesian meta-analysis suggests that cefepime treatment may increase the risk of mortality compared to other Γ-lactam antibiotics, although the effect is small and potentially mitigable. The determination is based on an analysis of 110 trials comparing cefepime to other Γ-lactam antibiotics, which included 22,608 patients.
The review, conducted by Zhara Sohani, MD, PhD, and colleagues, found that cefepime was associated with a 94.4% posterior probability of higher mortality compared to other Γ-lactam antibiotics. The analysis also found that the signal of increased mortality risk was most pronounced in trials of febrile neutropenia and severe bacterial infections, and with higher doses.
Cefepime’s Safety Profile
Despite the potential increased risk of mortality, the Infectious Diseases Society of America (IDSA) recommends cefepime as a first-line treatment for febrile neutropenia and infections from organisms with moderate risk of AmpC Γ-lactamase production. The IDSA’s recommendation is based on the drug’s efficacy and safety profile, which has been established through numerous studies.
As Daniel Usian, MD, MBA, and Ethan Smith, PharmD, note, “Cefepime is not a dangerous drug in search of a replacement; it is a useful drug in search of a dose.”
Bayesian Meta-Analysis
The use of a bayesian random-effects model in the analysis allowed the investigators to ascertain a probability of harm rather than a binary of statistical significance. This approach, which is closer to how clinicians reason, enabled the researchers to provide a more detailed understanding of the potential risks associated with cefepime.
The analysis found that cefepime was associated with a 98.6% posterior probability of higher mortality compared to other Γ-lactam antibiotics in 73 published peer-reviewed trials comprising 15,411 patients. The results suggest that cefepime may be associated with a small but significant increase in mortality risk, particularly in certain patient populations.
Implications for Clinical Practice
The findings of the study have implications for clinical practice, particularly in the treatment of febrile neutropenia and severe bacterial infections. Sohani and colleagues note that the results support a detailed discussion of cefepime’s safety in guidance statements.
In the middle of this ongoing debate, it is clear that the safety profile of cefepime is complex and involves many factors. While the drug has been shown to be effective in certain patient populations, its potential risks cannot be ignored. As researchers continue to study the effects of cefepime, clinicians must remain vigilant and adapt their treatment strategies accordingly.
